Alumni Engagement Reporting This form should be used to report university partners alumni engagement for inclusion in Ohio University's annual CASE Alumni Engagement Score. Reporting should follow the fiscal year (July - June) and be categorized as Volunteer, Experiential, or Communication engagement. Please submit using excel documentation only. Fundraising Clearance Form Download and complete this form when hosting a fundraising event. Email completed form to giving@ohio.edu or send via campus mail to the Office of Annual Giving, McKee House, Athens, OH 45701. Annual Giving

Payroll Deduction Form Return the form to the Ohio University Office of Annual Giving at giving@ohio.edu. The form can be submitted digitally without a signature if it is submitted as an email attachment from the employee’s OHIO email address. University Advancement-Request for Leadership Engagement This form is used to request meaningful and strategic engagement between donors or prospects and the Office of the President and/or the Office of the Vice President; should be filled out by an Advancement employee.
